• Medicare Annual Wellness Visits (AWV) for ages 65+
• Pediatric Well-Child Visits (for ages 2 weeks and up) and Medicaid EPSDT Examinations
• Pre-Operative/Pre-Surgery Exams
• Sports Physicals 
• Well Woman Gynecologic Exams
• Well Male Exams 
• Reproductive and Sexual Health 
• Hormone Replacement Therapy (HRT)
• Testosterone Replacement Therapy (TRT)
• Travel medicine and immunization counseling
• B12 injections
• Immunization/Vaccinations

Concerns regarding your hypertension, diabetes, thyroid issues, or other chronic health conditions? Efficient care for/management of chronic diseases is key to having a healthy life. Consistent checkups will allow our team to diagnose and manage many chronic problems, and prevent chronic diseases from adversely impacting your health and quality of life. 

Have a cough, runny nose, or fever? A sore throat, ear ache, or sneeze? How about pain after a fall or a minor injury? No worries- we've got you covered with *same day or next day visits so give us a call. We also offer minor procedures such as: 

• Ear Lavage/Irrigation for excessive ear wax buildup
• Staple/suture removal
• Skin tag/wart removal

 *Same or next day visits are based on provider schedule availability.

Mental health matters, and is a critical component to a healthy lifestyle! Our clinicians are trained to handle several mental health conditions such as acute grief reaction, anxiety, depression, PTSD, etc. If your needs extend beyond the scope of what our practice has to offer, we will make sure that we connect you with the right specialist(s) to take care of your needs. 

Are you struggling with weight gain or losing weight? Is your body mass index (BMI) out of range? Are you ready to lead a healthier lifestyle and don't know exactly where to start? Our team is skilled in obesity and weight management, and capable of assisting you with healthy weight loss options that are right for you.  

Options include (but are not limited to):

• Referrals for Bariatric specialists or surgeon

*Prescription weight loss medications are contingent upon normal physical exam and lab results

The following diagnostic imaging and testing services are available in the clinic:  

• ABI, Doppler/Extremity Studies (used to detect peripheral artery disease and issues with circulation in lower extremities/legs and feet)
• Electrocardiogram (ECG/EKG) and Echocardiogram (ECHO) for chest pain/cardiac concerns
• Spirometry (for breathing/respiratory disease concerns)
• TB Skin Testing
• Ultrasound

*An in-house lab technician is available to draw a wide variety of diagnostic blood tests (to be sent out to our lab partners for processing) to check for and/or diagnose several health conditions.

*An in-house imaging technician is also available by appointment for diagnostic imaging, such as Ultrasound. 

**Rapid diagnostic testing is also available in the clinic for the following conditions: 

• Respiratory viruses such as flu, COVID, RSV, and Strep
• Urinalysis to assess for urinary tract infection (UTI)
• Hemoglobin A1c to detect diabetes and how well it is controlled

*Turnaround time for send out labs varies; most results are received and reviewed by the medical provider within 24-72 hours.

**In house lab results are available the same day, typically within 15-20 minutes after testing is performed.

Can't physically make it into the office? No worries- we've got you covered via the convenience of telemedicine. The following medical services are now available via our telehealth platform:

• Sick Visits (*same day or next day appointments based on availability)
• Chronic Disease Management
• Medication Management (New Prescriptions and Refills of non-narcotic medications)
• Reproductive and Sexual Health Concerns
• Management of Mental Health Conditions (Anxiety, Depression)
• Travel Medicine/Immunization Advisement
• Obesity/Weight Management
• Laboratory Services
• Referrals

*Telemedicine visits may not be covered by your insurance provider. If your insurance plan does not cover this visit, you may be eligible for a self-pay discount. Please speak with our front desk or billing staff for more details. 

There are many benefits to Concierge Medicine/Direct Primary, such as:

• Pay a flat rate monthly fee and have all of your primary care needs covered without having to worry about deductibles and copays for each visit and/or procedure. *This includes in office/face to face and telehealth visits with the medical provider.

• Ability to Communicate with a medical provider 24/7, 365 days a year (via text, phone call, virtual telemedicine visit, secure messaging via patient portal or email *for non-emergent issues)

• *Evening appointments by request from 5pm-7pm (*speak with scheduling for more details)

• Housecalls (for Concierge Medicine clients who meet the eligibility criteria)

Completion of Pertinent Documentation such as: 

• Biometric Forms 
• FMLA and other approved leave forms
• Pre-Op Documentation
• Sports Physicals
• Referrals to appropriate medical specialists for care when applicable
